I hereby grant Micah's Visionary Photography permission to utilize my photographs and likeness in all media formats for purposes including, but not limited to, advertising, portfolio, demonstrations, trade, stock photography, editorial use, and artistic alterations, without restrictions. I understand that I am not entitled to any compensation for such use.

I release Micah's Visionary Photography from any claims or liabilities related to the use of my images and likeness. I confirm that I am over 18 years of age and fully comprehend the terms of this release.

Additional Notes:

• If the model is over 18, a signature from the model is sufficient.
• If the model is under 18, a signature from a parent or legal guardian over 18 is required.

Failure to obtain the necessary signature(s) will result in the inability to proceed with this agreement.
